Dear Parent(s)/Guardian(s),
Worth School District 127 is excited to announce that we will be offering free dental care for all of our students. We are partnering with a charitable organization called Mobile Care Chicago to offer no-cost dental services to your child. All services are provided at NO COST to you. Though you will not pay for any of these services, health insurance will be billed, if available. Services are optional.
A Full-Service Dental Van will be visiting our schools on the following dates:
- February 25, 2020 – Worth Elementary School
- February 26, 2020 – Worth Jr. High School
- February 27, 2020 – Worthwoods School
- February 28, 2020 (if needed)
The Dental Van is a dentist on wheels. On these dates, the dentists will perform a dental screening and provide preventative care in the form of an oral examination, cleaning, fluoride varnish and sealants. All students will be provided with a new toothbrush and toothpaste. The Dental team will fill out the State of Illinois Dental Form that is required for students in Kindergarten, 2nd and 6th grade. This exam is also required for students going into high school. If the dentists find that your child needs additional care, they will send home another form letting you know what needs to be done. They may offer a second consent form for permission to return to school to treat your child, or you may elect to take your child to your own dentist.
State law requires us to obtain your consent in order for your child to receive services from Mobile Care’s Dental Program. Please take the time to fill out the consent form here letting us know whether or not your child may receive services from Mobile Care’s Dental Program.
