Parent/Guardian Return to School Survey for 2020-21

Many significant decisions must be made for reopening Worth 127, and your input is important. Please answer the questions on the linked form regarding the start of the 2020-21 school year.

As you respond to the questions, please note that as of June 25, Illinois is in Phase 4 of the Restore Illinois Plan, and Worth 127 students are able to return to school for in-person learning.

In Phase 4, the Illinois State Board of Education (ISBE) and the Department of Public Health (IDPH) require that:
a. Masks or face shields will be required to be worn by all faculty, staff, and students.
b. Social distancing protocols must be followed as much as possible.
c. Extensive cleaning will occur throughout the day and in the evening.
d. Large group gatherings or activities over 50 people are not allowed to meet.

Please note that these requirements may change by the time school resumes, and Worth 127 will continue to update the Return to School plan as ISBE/IDPH guidance evolves.

The information gathered from this survey will be used to guide the reopening of schools.
